The Costa del Sol has some of the best private medical infrastructure in continental Europe — and a public system that, while busy, is genuinely good. For most UK retirees the practical setup is: register for the public system via the S1 form (if you're entitled), and carry a private policy for speed, choice and English-speaking specialists.

The S1 form — free public access for UK pensioners

If you receive a UK state pension, you can apply for an S1 form from the NHS Overseas Healthcare Service. Registered with the Spanish system (INSS), this gives you free access to public healthcare exactly as a Spanish resident would have. The S1 must be issued before you become tax-resident in Spain to be straightforward.

The private layer

Most UK residents and retirees carry a private policy on top of the public system, for three reasons: faster appointments, English-speaking specialists, and choice of hospital. Typical 2026 costs:

  • Age 50: €60–€90/month for full cover
  • Age 65: €120–€180/month
  • Age 75: €250–€350/month (some insurers cap entry at 70 — check before you move)

The major insurers: Sanitas, Adeslas, DKV, Asisa, Allianz.

English-speaking GPs

Plenty exist, particularly within the private networks. Both Helicopteros Sanitarios (a Marbella institution offering 24/7 home visits) and many Sanitas / Adeslas clinics have permanent English-speaking GPs.

Prescription costs

If on the public system via S1, prescription co-pay is 10–60% depending on income (pensioners pay less). If on private only, you pay full price — but most Spanish generics are dramatically cheaper than UK retail equivalents.

The dental and optical gap

Neither public Spanish healthcare nor most private policies cover routine dental or optical. Standalone dental insurance from Sanitas / Adeslas runs €15–€25/month. Out-of-pocket cleanings are €40–€80; implants €1,200–€1,800 each — significantly cheaper than UK private equivalents.

What to set up in your first month
